sukuna true form is the body Ryomen Sukuna had in Japan's Heian era, with two faces and four arms, based on the two-faced figure Ryomen-sukuna in the Nihon Shoki. Sukuna's true form is the body he had during the Heian era, with two faces and four arms, and it finally shows up at the end of chapter 237. The anime has not touched it. The design traces back to Ryomen-sukuna, a two-faced figure in Japan's oldest chronicles.
Sukuna's true form is the body Ryomen Sukuna had in Japan's Heian era, more than a thousand years ago, with two faces and four arms, based on the two-faced figure Ryomen-sukuna in the *Nihon Shoki*. Most anime viewers have never seen it, because the Sukuna on screen so far is a curse wearing someone else's body. Where it fits in the whole Jujutsu Kaisen story is near the very end.
Spoiler line: this post spoils Sukuna's takeover of Megumi (chapter 212), the result of the Gojo duel (chapter 236), the true form (end of chapter 237) and Sukuna's defeat (chapters 267 and 268). All of it is manga only. The anime stopped around chapter 181.
Pick your path: what it is, the chapter, the anime, why he looks like Yuji or his face markings.
YourCrush.AI has Sukuna as a chat character, so once you have read this you can talk to Sukuna yourself.
What is Sukuna's true form?
His original Heian-era body: two faces, four arms. The design traces back to Ryomen-sukuna in the Nihon Shoki, and "Ryomen" literally means "two-faced".
Wikipedia's Sukuna article gives the two faces, the four arms and the Nihon Shoki source. CBR adds what the manga page shows: four eyes, four arms and two mouths, one of them on his abdomen, and a body covered in tattoos much like the ones that appeared on the bodies he took over. The Sukuna true form design takes a figure from a Japanese chronicle and turns it into a character: a sorcerer named after a being the old texts describe as two-faced.
Our imagining of Sukuna in the Heian era, on being called two-faced:
"They whisper it as though it were an insult. Two faces, and both of them looking at you. Count again."
How YourCrush.AI lets you design a character with a second face and a true form of their own
A true form lands as a reveal because the reader met the mask first. You can build that kind of character from scratch, an original one, and decide for yourself when the second face shows.
YourCrush.AI builds a character from a description and a separate backstory, so what the character shows you and what they really are can live in different fields. YourCrush.AI drafts those fields for you from a name and an image if you want a starting point, and generates images in chat when the story needs one. YourCrush.AI branches a story, so you can play the night the mask comes off more than one way. It is free to join and 18+ only.
Here is an original adult character, inspired only by the two-faced idea, to build at the create-character page in the natural behaviour mode:
- Name: Kotaro Mibu
- Behaviour mode: natural
- Category: Anime
- Description: Kotaro is 45, a mask carver in a mountain town whose masks always have two faces, one on each side. He works in a cold workshop full of cedar shavings, speaks rarely and exactly, and watches your hands more than your face. Locals say he wears a mask himself and nobody has seen which side is real.
- Backstory: You commissioned a mask from him a year ago and almost forgot about it. He finished it today, and it has your face on one side and a face you do not recognise on the other. He sent for you before anyone else could see it.
- Greeting: "It's done. Before you take it, turn it over. Yes, that side too. People always ask whose the second face is, and the answer is always the same: it's the one you came here hoping I wouldn't carve. Sit. You owe me a story for it, not money."
- Traits: Mysterious, Calm, Sarcastic, Intelligent, Cold-hearted
- Speaking style: low, sparse, wry, exact
What chapter does Sukuna show his true form?
Chapter 237, at the very end, during his fight with Kashimo right after he kills Gojo. Some secondary sites list chapter 238 instead, but that's the chapter confirming Kashimo's death, not the transformation itself.
The reveal comes from Game Rant. It lands in the middle of the Shinjuku Showdown, at the end of chapter 237, with Gojo dead one chapter earlier.
Spoiler: the true form revealed (ch. 237)
Hajime Kashimo has waited four hundred years for this fight. He steps in the moment Gojo falls and spends his one-time technique, Mythical Beast Amber. At the end of chapter 237, in front of him, Sukuna changes: the body he fights in becomes his own Heian body, two faces, four arms. Kashimo's one-time technique is not enough. He dies to the same net of slashes that killed Gojo, and chapter 238 confirms it.
Gojo never faces this form. He dies in chapter 236, and the duel between Gojo and Sukuna is fought entirely against Sukuna in a borrowed body.
Why did Sukuna not take his true form earlier?
Because he had deliberately paused his own incarnation, and he could resume it only once. He held it back until his Reverse Cursed Technique had burnt out against Kashimo, keeping it as a last card.
Sportskeeda's chapter 237 coverage calls it a transformation "that he has purposefully halted before and that can be made without Reversed Cursed Technique only once", though that wording rests on a single outlet. Game Rant calls it a held-back "trump card" he would have played sooner if Maki and Yuta had joined Gojo against him. Resuming the incarnation restores his original body and heals him fully, according to CBR. The groundwork went in before the duel: in chapter 222, he ate the mummified corpse of his original body, along with the fingers Uraume had gathered.
Is Sukuna's true form in the anime?
No. Season 3 wrapped up inside the Culling Game, somewhere around chapter 181, and Season 4 still has no release date. That means the Shinjuku Showdown and Sukuna's true form have not been animated yet.
Season 3 ran from 9 January to 27 March 2026, per Wikipedia, and Screen Rant puts its stopping point around chapter 181. Anime News Network reports Season 4 is in production with no release window. The August 2026 key visual shows a red, demonic hand reaching out of the shadows. That's the closest the anime has come so far.
What did Sukuna look like before Yuji?
Before his power was sealed into 20 fingers, Sukuna was the strongest curse user of the Heian era, and he wore the two-faced, four-armed body that his sukuna true form brings back. The Sukuna most anime viewers know is that same curse walking around in Yuji's body.
Recap: who Sukuna was before Yuji
A thousand years ago, Ryomen Sukuna was the most powerful curse user in Japan, served by Uraume. His power ended up sealed in 20 mummified fingers, scattered and hidden. In chapter 1, a high-school boy, Yuji Itadori, swallows one to save his friends, and keeps control of his own body. Every Sukuna you see in the anime is that curse using Yuji.

Our imagining of Sukuna on what it is like to be cut into twenty pieces:
"Twenty fingers in twenty boxes, and every one of them still mine. You did not scatter me. You only made me patient."
Why does Sukuna look like Yuji?
Because for most of the story, the Sukuna you see on screen is Sukuna in control of Yuji's body. That's why his listed height, 173 cm, matches Yuji's exactly. From chapter 212 he moves into Megumi's body instead, and the sukuna true form only appears after that.
The matching height comes from the age and height tables press outlets compile, and it is a vessel's height. The two are also related by blood, which is a separate question: see whether Sukuna is related to Yuji.
Spoiler: why Sukuna leaves Yuji's body (ch. 212)
Yuji's body is a cage Sukuna does not control: he is sealed inside it by an unbreakable vow. So in chapter 212 he uses Enchain, a binding vow that gives him one minute in control. He tears off a finger, turns it into a cursed object and forces Megumi to eat it, moving into Megumi's body. He wanted the Ten Shadows, and he uses them in the duel with Gojo.
What do Sukuna's face markings mean?
Nobody outside Akutami knows. The author has never confirmed what the tattoos symbolise. What canon does show is that thick black lines appear on the host's face when Sukuna takes control, and his true form's whole body carries the same kind of markings.
Game Rant describes the lines running from under the lips up to below the eyes, across the nose and in a pattern on the forehead, and quotes the plain fact that Gege "has never exactly confirmed the exact symbolism". Theories that they echo Heian-era criminal tattoos or amplify cursed energy are fan and press speculation, not canon.

How tall is Sukuna's true form?
There's no official height for Sukuna's true form in any source I could verify. The 173 cm figure that shows up in character profiles is his height in Yuji's body, not the Heian form.
So treat any number you see for the four-armed body as a fan estimate. The height tables list Sukuna and Yuji at the same height because they're describing the same body.
What happens to Sukuna's true form in the end?
Chapter 267 is where it ends. Yuji's Black Flash tears Sukuna out of Megumi's body, and what comes out is a weak, blob-like thing. By chapter 268 he disintegrates. He is still inside Megumi when the true form takes shape, and that form does not survive the last fight.
Spoiler: what is left of Sukuna at the end
Yuji's Black Flash forces Sukuna out of Megumi's body into a weak, blob-like form in chapter 267. In chapter 268 Yuji offers to be his vessel again so he can live. Sukuna refuses and disintegrates. One of his fingers, kept at a shrine as a protective talisman, is the manga's final image.
