Who killed Toji?
Satoru Gojo killed him in 2006, at the end of the Riko Amanai job. In Shibuya nobody kills him: Toji ends it himself (Wikipedia, Season 2).
Toji's first death is the end of the job he took to kill Riko Amanai. The Time Vessel Association, a religious group opposed to Riko's merger with Master Tengen, hires him to do what two teenage sorcerers are guarding her against.

Back in Tokyo, Toji ambushes Gojo, Geto and Riko. Using a special cursed tool, he apparently kills Gojo. Then he fatally shoots Riko in Tengen's lair (episode 3; Wikipedia, Season 2).

Toji has beaten Geto, delivered the body and collected his fee. The job is done. Then the boy he killed walks back in.

Gojo survives by activating Reverse Cursed Technique, comes back, and uses Hollow Purple, which destroys Toji's left side (chapter 75, episode 4, overall 28). Dying, Toji tells Gojo that his son Megumi will be sold to the Zen'in clan (Wikipedia, Season 2; Sportskeeda).

How did Toji come back?
Granny Ogami, a curse user, uses her Séance Technique to summon Toji into her grandson's body in Season 2 episode 11, "Seance" (overall 35) (Wikipedia, Season 2). The technique pulls up the body's information only, never the soul. Toji's Heavenly Restriction physique is what lets his consciousness overwrite the grandson's soul anyway (Dexerto).

He seizes control of the grandson's body and kills Granny Ogami in episode 12. From there he picks up Maki's cursed tool and exorcises Dagon, which collapses Dagon's domain in episode 15. Then he throws Megumi out of the building and overwhelms him across episodes 15 and 16 (Wikipedia, Season 2; Game Rant).

Inside Dagon's domain, Nanami, Maki, Megumi and Naobito Zen'in are barely holding on. Toji walks past all of them. He only cares about the strongest presence in the area.

Why that answer and not another? Toji's life was built on spiting the Zen'in clan that rejected him, and CBR calls his whole profession an attempt to spite them (CBR). He had arranged to sell Megumi to that same clan, which the anime frames as Toji believing it would give his son a better future (Wikipedia, Season 2). Our reading: when Megumi says "Fushiguro", Toji learns the sale never took, and that is the one thing that makes him stop. Megumi never learns in Shibuya who his opponent was (Dexerto).

Is Toji dead for good?
Yes. The possession ends when he stabs his own vessel in episode 16, and the sources we use report no later return (Wikipedia, Season 2; DualShockers).
